As a key member of the interdisciplinary health-care team, you will apply your knowledge of drug therapy in addressing patient drug-related problems, as you provide Pharmaceutical care to selected patient groups through the contiuum of their treatment course. As well, you take an active role in the education of Pharmacy residents and students. Qualifications Registered as an “A” Registered Pharmacist with the Ontario College of Pharmacists Successful completion of a Honours B.Sc. Pharmacy Degree is required Hospital pharmacy residency certificate is preferred Pharm D from an accredited school required for the Oncology Pediatric Inpatient (CHWO) Pharmactist position Ability to work in a computerized setting (previous experience with Kroll Pharmacy Computer systems is preferred) A high degree of accuracy and attention to detail are essential Excellent problem solving and decision making skills Effective organizational, communication and customer service skills Must demonstrate strong ethics and integrity Ability to interact effectively with patients and other health care professionals Must be proficient in NAPRA Professional Competencies, CSHP Standards of Practice and OCP Standards of Practice Ability to work with minimal supervision and initiative to work independently as an effective team member Demonstrated ability to attend work on a regular basis
