  • Master’s Degree in Social Work from an accredited university
  • Registration in the College of Social Workers and Social Service Workers
  • Recent clinical experience in an acute health care setting
  • Preference will be given to applicants with recent Family Medicine/Palliative Care experience who demonstrate a strong understanding of psychosocial issues re: illness and bereavement
  • Palliative Care Level I and Level II /Fundamentals preferred
  • Ability to function both independently and interdependently within an interdisciplinary health care team
  • Highly developed assessment, treatment and case management skills
  • Demonstrated strengths in interpersonal relations, problem-solving, communication, group dynamics, community development and other innovative approaches
  • Demonstrated leadership in discharge planning and community networking resources
  • Demonstrated skills in providing education to health care team and to multidisciplinary students
  • Demonstrated ability to measure and evaluate practice change outcomes
  • Supports research and innovation in practice
  • Ability to support and assist health care teams facing change and stress in environment
  • Willingness to participate in Celebration of Life ceremonies periodically
  • Demonstrated knowledge of and commitment to patient and staff safety at LHSC
  • Demonstrated ability to attend work on a regular basis
